Infinite Health Collaborative (iHealth) Breast Health Nurse Navigator - OBGYN Associates in Edina, Minnesota

Description The Nurse Navigator will provide navigation and education services to our patients in a professional and skilled manner. This is a part-time position that will potentially support all of the Women's Health Clinics across locations. OBGYN Associates is an operating division of Infinite Health Collaborative (i-Health), a group of local medical practices, owned and led by its physicians, on a mission to give patients more control over their healthcare. Essential Functions: Provide comprehensive care coordination for women who require breast health examinations, screening and diagnostic evaluations of breast tissue through scheduling of appointments, providing to patient follow-up of results and facilitation and connection with specialists for follow-up care as indicated Coordinate care utilizing standard, evidence based clinical pathways for breast health program Provide ongoing physical and psychosocial support for women undergoing breast health interventions through face to face connections, teleconference calls, phone calls and other methods of communication Perform outreach interventions for women within the women's health division who are eligible or meet criteria for breast health surveillance activities such as provider exams, screening mammograms etc. Manage and coordinate the care of women undergoing follow-up and treatment for breast abnormalities through the development of a comprehensive, individualized care plan that includes all specialist care recommendations, treatment and surveillance interventions Collaborate closely with specialist patient care navigators to establish continuity of care and oversight of the women's healthcare journey Participate and assist with coordination of Breast Awareness events and recognition within the women's health division and in the community Collaborate closely and frequently with the patient's OBGYN provider regarding care coordination activities and patient needs, and provide regular and timely updates to the provider regarding the patient's clinical status and specialist treatment plans Triage patient concerns appropriately and timely to the OBGYN provider Documentation of navigation activities, connections, follow-ups needed, and education completed in case navigation software Collaborate on the development and delivery of individualized and diversified educational materials and products for women around breast health awareness, follow-up and treatment of abnormalities, prevention of complications, and survivorship Collaborate on the data collection and analysis process through: Accurate and timely documentation in case management software Data collection for entities both within and outside of the women's health division Early and timely identification of trends and triage appropriately to leadership Participation in larger group discussions about outcomes data and analyses Implementation of changes or additions to the model or framework for the pilot Answer questions and concerns from patients in coordination of care team Any and all other duties as assigned Education and Experience Requirements: Registered Nurse license in the state of Minnesota or the ability to obtain license in the state of Minnesota by date of hire required. Bachelors degree in an accredited nursing program. Minimum of three (3) years' experience as a Registered Nurse required Labor & Delivery experience required 3+ years experience in Women's Health preferred Current RN licensure in good standing with MN Board of Nursing, no lapses Essential Requirements: Comply with company policies, procedures, practices and business ethics guidelines.
