Insight Global Housing Case Manager in Hartford, Connecticut

Job Description

Insight Global is looking for a Housing Case Manager. This position will assist project-based and scattered-site tenant-clients by providing services and referrals for a variety of needs including general assistance, substance abuse counseling/programs, mental health, medical care, employment services, etc.

Environmental and Working Conditions:

This position works in an office providing case management to clients, as well as traveling to client apartments either in project-based buildings or scattered-site apartments within Greater Hartford to make client contacts. Evening hours may occasionally be needed based on client availability.

Job Responsibilities:

  • Make referrals for general assistance, benefits, medical care, mental health, community agencies/eligible services, substance abuse counseling/programs, employment services, etc.

  • Educate and assist clients in daily living skills

  • Assist clients with supportive services and/or other types of assistance as needed

  • Maintain a visible presence and contact with clients

  • Successfully engage clients in various areas, including with in-house and external group work to enhance recovery, mental and physical wellness, etc.

  • Improve client access to services

  • Inform clients of upcoming events

  • Maintain up to date client files

  • Meet all quality standards set internally and by funders and other partners

  • Ensure complete data is entered and accurate in HMIS database

  • Produce reports as needed in HMIS for housing utilization, etc.

  • Communicate important daily events and other information

  • Maintain daily statistics

  • Complete progress notes for all client contacts

  • Attend and participate in staff meetings

  • Be knowledgeable regarding resources and issues affecting our target population

  • Return all phone calls, emails etc. within 24 business hours

  • Other duties as needed/assigned

Physical and Mental Requirements:

Must be able to perform diverse job tasks including manual labor (lifting up to 40 pounds), clerical work and case management. Individual should show initiative, be open to suggestions and learning, an independent worker who knows how to be part of a team, and someone who can listen with their heart and then logically pursue available options for assistance. This position requires the ability to be pleasant, patient, compassionate and consistent with clients. It is beneficial to have experience having worked with people who are homeless, or others who are of low income, low educational level, or are mentally or physically impaired.

Skills and Requirements

  • Bachelor's degree in Human Services field or 3-5 years relevant experience

  • Commitment to Housing First, Harm Reduction, Person-Centered, and Consumer-Choice models of care

  • Excellent interpersonal and written communication skills

  • Compassionate concern for those who were homeless or are at risk of becoming homeless

  • Valid driver's license

  • Ability to handle stressful situations and a fast-paced environment

  • Experience with computers and proficient typing skills

  • Ability to complete necessary paperwork and maintain client files

  • Tenacity for providing on-going support, follow-up, and client advocacy

  • Knowledge of Microsoft Office 365 suite Bilingual null
