HCR Home Care Oasis Reviewer - Fulltime - Hybrid Option Available in Rochester, New York

Role and Responsibilities

The OASIS Review Specialist is responsible for conducting quality assurance documentation reviews of assigned records for accuracy and ensuring they comply with regulatory and OASIS guidelines. This would include making recommendations to submit accurate OASIS assessments. This role would also work with staff in education of OASIS guidelines.

Essential Functions

  • Review OASIS assessments to ensure appropriateness, completeness, and compliance with federal and state regulations and organization policy

  • Consult with appropriate clinical staff to clarify any data integrity issues and works with clinician to make appropriate corrections per organization policy

  • Work with managers to address trends that affect the agency's outcome and process measures noted during OASIS review

  • Assist with other chart audit activities as assigned including reviewing identified areas of patient medical record for completeness and consistency

  • Meet or exceeds established productivity and timeliness expectations.

  • Work as a team with co-workers to meet departmental and agency goals.

  • Conduct education sessions with staff (1:1 or group setting) to inform of documentation deficiencies, provide corrective action as assigned and provide staff training for new OASIS initiatives

  • Participate in the development of Quality Assurance and Performance Improvement (QAPI) Program.

  • Maintain professional and technical knowledge by attending educational workshops and reviewing professional publications

  • Maintain certifications OASIS certification

  • Establish and promotes positive, ongoing communication with company personnel.

  • Other duties as assigned.

    Qualifications and Requirements

  • OASIS Certification (COS-C) or (HCS-O) required

  • Excellent understanding of OASIS including at least two years of experience reviewing OASIS documentation for quality assurance

  • Excellent understanding of home care regulations

  • Demonstrates excellent verbal, written, and critical thinking skills; must be able to be proactive and work well under pressure to meet deadlines and company needs.

  • Demonstrates organizational and multi-tasking skills, as well as professionalism and discretion, with the ability to get along well with others in a work environment; must also demonstrate the ability to work independently; must be proactive, responsive and resourceful.

  • Proficient in basic computer skills including Word and Excel.

  • Must be comfortable learning new electronic medical records.

  • Must have current driver license and reliable transportation.

    Work Environment

The OASIS Review Specialist is primarily in an office setting and may be exposed to outdoor conditions.

The working conditions are classified as sedentary work:

  • Sedentary work - Exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time.
